Walker Community Care provides financial assistance for in-home care services. Funding amounts are determined using a tiered scale based on the applicant's financial profile and the specific level of care required.To be considered for funding, applicants must complete the full application process detailed below. Incomplete applications will not be reviewed.

WCC conducts an initial assessment based on financial need and current insurance coverage. Applicants must submit the following supporting documentation alongside their application:

Tax Documentation: A copy of your most recent federal Form 1040 tax return.

Proof of Income & Expenses: Statements from the last three (3) consecutive months for all active bank accounts, clearly showing regular income and household expenses.

Insurance Policies: A copy of any current long-term care insurance policies (if applicable).

In-Home Care Agency Agreement: A copy of your official service agreement with your chosen provider.
Note: WCC reserves the right to review and verify that the selected agency is an independent, qualified company specializing in professional in-home care.

If an application meets the preliminary financial criteria and is accepted for further determination, WCC will schedule an in-home evaluation.
This visit is designed to collect both objective and subjective data regarding the applicant’s specific needs. The evaluation will assess key areas, including but not limited to:

Activities of Daily Living (ADLs)
-
Mobility and ambulation
-
Home safety and environment
-
Emergency care assistance needs

Once the home evaluation is complete, the entire application packet, incorporating financial metrics, insurance status, and care assessments, will undergo a comprehensive review. WCC will make a final determination, and the applicant will be formally notified of the decision in writing.

WCCa will only provide up to fifty percent (50%) of your in-home care cost
wcc does not provide the in-home care service for you. we are merely an assistance for funding.
wcc provides funding for non-medical in-home care. no skilled nursing services will be covered.
